I am coming across instances with certain OMS systems where this field is set always to 0.

Should this be rectified prior to sending the message, other than for status messages?

Or in practise is the uniqueness of this field not adhered to.

ExecID should be unique (except for status). I, personally, would make my counterparty rectify such a flaw.
