Does anyone know if a broker sends unsolicited executions, can the institution follow them with a cancel/replace request on the order?

There’s no technical reason why not. The institution would have to populate ClOrdID and OrigClOrdID in addition to the broker’s OrderID. As long as both parties agree, this shouldnt be a problem.
However in my experience, once an order starts life as a telephone order (aka unsolicited), it remains a telephone order.

But anyone know what’s the ClOrdID for Unsolicited executions. I know for bloomberg, sell side is not required to send this tag. But is it a universal standard?

Per the spec, ClOrdID is "not required for manually entered orders" in the Execution Report message definition.
