It was an oversight in the 4.2 spec to omit GTBookingInst from the New Order - Single message.
The buy-side is likely the firm specifying it, and would do so in the order.
I believe this will be fixed in the upcoming errata to FIX 4.2. While we generally do not like to add new fields to messages in errata, the fact that this feature was approved for 4.2, dropping the field was an error, and the feature cannot be implemented without adding it will likely cause it to be included in the errata.
