The PDF contains only 28 of the 39 pages. Appendix 3 is missing completely.

The FIX Program Office has reached out last week to the co-chairs of the FIX Reference Data Working Group that produced this document. They have not heard back and will follow up.

The complete document has now been posted. Apparently, the PDF converter ignored Appendix 3 due to its landscape format. There was no intention to withhold that information.
